Notice: Reference/ Background/ Criminal History Checks


  • I understand that any offer of employment is conditional on a satisfactory reference and background check, which will include but is not limited to a criminal history record check.
  • I authorize the company to contact my former employers, references, and any and all other persons and organizations for information bearing upon my qualifications for employment.
  • I further authorize the listed employers, schools, companies, credit bureaus, government agencies, and personal and professional references to disclose to the Company (without further notice from me) any and all information about my background, character, general reputation, previous employment, education, and criminal record, along with any other pertinent information they may have.
  • I release such parties from all liability for any damage that may result from furnishing same to The Company, and I release The Company and its agents, from any and all claims, liabilities, or demands arising out of or related to such investigation, disclosure, or release of information.


Notice: Drug Testing

  • If I am conditionally offered employment, I understand that I may be required to submit to a pre-employment medical examination and/or drug test before starting work. If employed, I also agree to submit to a medical examination and/or drug test at any time deemed appropriate by The Company and as permitted by law. I consent to such examination and tests, and I request that the examining doctor disclose The Company the results of the examination, which results shall remain confidential and separate from my personnel file.

Notice: Personal Information Release

  • All Care Consultants frequently receives requests from our clients to receive your personal information at the time of submission for consideration for an interview. Our clients have indicated that prior to interviewing, a candidate’s personal information will be used to verify eligibility with Medicaid Reporting and other such entities as they require for eligibility for interview.
  • All Care Consultants policy is not to release your personal information to any party outside the All Care Consultants organization. In order for us to vary from that policy we request you authorize and acknowledge the release of the information below.
  • I have read the above request for release and I do authorize the release of my personal information.

Applicant's Statement

  • I understand that if I am offered employment by the Company, I will be required to attest to myidentity and employment eligibility, and to present documents confirming my identity and employment eligibility. I understand that I cannot be hired if I do not comply with these requirements.

No promises regarding employment have been made to me, and I understand that no such promise or guarantee is binding upon the Company unless made in writing.I understand that completing this application does not indicate there is a position open and does not obligate The Company to hire.I agree to abide by all company work rules, policies, and procedures.
